Informed Consent for genetic service

By ordering MyRISQ services you confirm that you have requested this service after careful consideration of the following facts, which are provided to ensure that I understand the implications of gene testing:

  • The genetic test will only screen for specific genetic alterations that may affect your clinical management in relation to disease diagnosis, medical treatment and/or lifestyle modification.
  • Failure to detect a specific genetic alteration (negative test result) does not exclude the presence of undefined gene mutations or other risk factors not tested for.
  • Detection of a genetic alteration (positive test result) in DNA extracted from a blood sample or cheek swab implies that close relatives may also have the same genetic change(s), while tests performed on tissue biopsies (e.g. tumour to determine cancer prognosis) only relates to your health without any implications for other family members.
  • A positive genetic test does not mean that the person has a genetic disease or will develop the condition, but it may indicate increased risk of disease in the absence of appropriate risk reduction intervention.
  • Early detection or pre-clinical diagnosis of treatable or preventable genetic diseases (e.g. inherited iron overload) is beneficial.
  • Genetic testing may lead to better treatment and lifestyle choices, or possibly anxiety when certain genetic risk factors are identified in an individual without a family history or clinical symptoms of a disease.
  • The genetic material may be tested at different laboratories or using different mutation detection methods as part of a quality assurance process.
  • In the event that genetic testing is performed within a family context, non-paternity may be revealed and it is therefore important that non-biological relationships (adoption) be reported at the time that specimens are obtained for genetic testing.
  • Identification of genetic alterations in individuals with disease symptoms or a strong family history will under no circumstance be forwarded to insurance companies.
